If using nuts, place in a 300°F oven for 3 - 5 minutes just until lightly toasted. Remove from oven and set aside.
Turn oven temperature up to 350°F and line baking sheet with parchment paper.
In a microwaveable bowl (large Pyrex works perfectly), add the Sweetened Condensed Milk, Chocolate Chips and Butter together. Place in microwave on high for 1 minute, stir and microwave again if needed until Chocolate has melted. Mix well.
Add Flour and Vanilla to melted Chocolate mixture and blend well. If using nuts, gently fold them in.
On a parchment lined baking sheet, drop 1 Tablespoon of cookie batter 1 - 2 inches apart.
Bake at 350°F for 7 minutes. Remove, let cool on baking sheet for 2 minutes then transfer them to cooling racks to continue to cool.
Keep in an airtight container up to 1 week.
